New Patient Information

Welcome to Laura Jozewicz, MD, PA! We look forward to providing you with the best neurological care. To schedule an appointment please call 919.845.2580. Please remember to bring the following to your appointment:

• Your insurance card
• Your current prescription and non-prescription medications you take
• Your medical records and information about your medical history
• Referral forms, if required by your insurance
• Phone number and address of the pharmacy you will be using

Please arrive at least 30 minutes prior to the scheduled time for your first appointment. This will allow you to complete all the necessary paperwork and complete your visit in a timely manner. If you would like to preview the paperwork please download and print the “PT Information” and “New PT History” forms. If your new appointment is for headaches, please review the “Headache Questionnaire” form before the appointment. If your new appointment is for sleepiness, please review the “Sleepiness Test” form before the appointment.
